Identify and compile (3) "hazards"; one from each of the three categories listed in NFPA 1600 (e.g., natural, human-caused, and technological). There is an expanded list of hazards in Annex A. The hazards that you select should be ones that could cause a significant impact or impacts to your organization (e.g., injuries/death, property damage, business interruption, environmental contamination, and or damage to brand/image/reputation/financial strength).

The “Overall Hazard Rating” is a two-letter combination of probability and impacts. For this example, the highest impact would be used (e.g., “LM” would be low probability and high impact). There are many variations of this table including many ways to quantify the “overall” hazard rating.
