Objective: The objective of this assignment is for the student to demonstrate the ability to organize and describe case material using cognitive and behavioral theories.
Assignment: The student should identify a case, either from their field work or some other helping role to use for the paper. A brief bio-psycho-social-spiritual summary of the case should be provided (2-3 pages) that includes the presenting problem of the case as if it were being placed in a medical chart. Please see the guide for this summary that is placed under Assignments. Using some of the examples used in class as a guide the student should write up a case formulation in a narrative form (i.e. do place the cognitive conceptualization chart in chart form). First, explain the target issue using behavioral theory, including the terms and concepts of the theory. Second, explain the target issue using a cognitive theory lens, also using the concepts and terms of the theory. Then using the combined CBT approach, describe the case in a NON-JARGON/CLIENT FRIENDLY manner (can be written as a summary to a caregiver or colleague or as if you were speaking directly to the client). In the CBT section, the connection between the thoughts, feelings, and behaviors should be clear as the student explains why the person is presenting with target issue (either a feeling, thought, or behavior) in the way that they do to the social work intern.This paper should be no more than 7-8 pages.

Objective 2: AEB a reduction in Bob’s scores on the Criterion D (negative alterations in cognitions and mood) subscale of the CAPS-5 Past Week questionnaire, within 14 weeks.
• Therapist will administer the CAPS-5 Past Week questionnaire at the start of sessions 2, 4, 8, 12, and 14.
• As part of prolonged exposure therapy (PE), Bob and therapist will engage in imaginal exposure. In session 3, therapist will thoroughly explain imaginal exposure to Bob, followed by conducting imaginal exposure: Bob will imagine himself reliving his traumatic experience, and will be asked to provide descriptions of the experience—with associated thoughts, feelings, and physical sensations—in as much detail as possible. Imaginal exposure may be conducted in each subsequent session and may also be tasked as homework, between sessions, where Bob will listen to an audio recording of the imaginal exposure.
